Introduction:

Acute appendicitis is the most common presumptive diagnosis in patients with abdominal pain in an emergency department. The use of imaging has decreased the percentage of white appendectomies. The objective is to integrate the management of routine diagnostic clinical scales in the approach of probable acute appendicitis and to look for predictive factors that can avoid a white appendix.

Objectives:

To integrate the management of routine diagnostic clinical scales in search of predictors of white appendage.

Material and methods:

A retrospective, observational, descriptive, longitudinal study was conducted to look for factors that contribute to an outcome of a white appendectomy in a private health services institution during 2011 and 2015. Laboratory, cabinet and symptomatology studies were analyzed. , frequently presented in the appendiceal charts and their relation with respect to results by pathology of the surgical piece.

Results:

A 35% of white appendectomies were reported, with predominance in women 73.81 vs 26.19% in men. The most significant variables were; sex (OR 5.955, 95% CI 1.494-23.735) p = 0.01, migration of pain (OR 36.20, 95% CI 6.78-193.32) p = 0.0001, leukocytosis (OR 14.102, 95% CI 3.45-57.61) p = 0.0001.

Conclusions:

The absence of migration of pain and leukocytosis are predisposing factors for the outcome of the white appendix. The female sex was predominant, so their previous diagnostic approach must be more rigorous.
