The decentralization of health services at the antioqueho southeast region at Colombia is analyzed in this paper, starting from some social actors perceptions regarding to the effects of such process promoted by the state and strengthened from the reform of Social Security General System of Health in 1993. It is to project the existing contradiction between the claim from the normatively formulation to create new places of actor's participation and recognition of the perseverance of the same traditional instances of social participation. Likewise, it is seen some incoherence, first, between the recognition proposal of the universal right to health through the participation of public institutions and the selection of beneficiaries which just the state can give the service, and second, the logical market conversion of the public hospitals of social state enterprise and of the devolvement to private ones a part of health attention.
