The aim of this systematic review was to evaluate prospective studies on medication prescription errors in adult inpatients. All articles were electronically searched in MEDLINE/PubMed®, EBSCO® and CINAHL® from 1900 threw May 2013. Findings were included in treatment lines in computerized prescription order entry and old fashion or hand written medical records. Medication errors were detected by direct observation. The percentage of error was between 1.5% and 34%. The main causes of medication errors were analyzed, as well as the initiatives for the reduction of such errors. The implementation of computerized prescription order entry in hospitals is an effective tool to offer quality in health care by lowering the medication prescription errors.
