Abstract

Introduction:
The POEM technique has shown similar results to Heller’s myotomy with partial fundoplication (MLH). One of the main drawbacks of POEM is represented by gastroesophageal reflux. The cardial constriction technique seeks to increase the tone of the lower esophageal sphincter, thus decreasing the passage of gastric contents into the esophagus and consequently improving the quality of life of the patient.
Objectives:
To demonstrate that cardial constriction is a less invasive and effective method, with results similar to anti-secretory therapy.
Materials and methods:
Retrospective, cross-sectional, descriptive study in three post-operated patients of POEM who presented gastroesophageal reflux refractory to medical treatment in the gastrointestinal endoscopy department of the Regional Hospital 1º Octubre, ISSSTE.
Results:
23 patient were analyzed with a history of POEM from 2016 to the current date. It was identified that 69% of the population were women with an average age of 57 years. Normal values were found in high resolution manometry 9 (39.1%) patients of LES and PIR in 18 (78.26%) patients, 6 patients had anti-reflux surgery through fundoplication, 9 patients had endoscopic data compatible with esophagitis, 3 patients refractory to treatment in which the cardial constriction was performed, finding an increase in the values of the length of the LES and the PIR but remaining with symptoms for GERD.
Conclusions:
cardial constriction is a new technique that should be more studied, to really assess the effectiveness.
