The intratecal reservoir is an effective alternative in the treatment of cancer pain.

Objective:

To describe the installation and application of the intrathecal reservoir for the management of the analgesic cancer pain.

Case:

A male patient, 52 years old with diagnosis of pelvic Ewing ́s sarcoma. He was treated by the oncology service with morphine, dexamethasone, gabapentin, paracetamol, metamizol and dexketoprofen of rescue with poor results. The Hospital Vall d ́ Hebron in Barcelona, Spain, was consulted because the pain in the right hip 10/10. An intrathecal reservoir was placed with external administration of morphine 35 mg with bupivacaine 0.5%, 25 mL plus 67 mL of physiological solution to 0.20 mL/h (morphine 1.68 mg/day). The clinic pain does a follow-up to get the suspension of intravenous analgesia and to achieve only contribution of analgesia through epidural and let the patient go home.

Conclusion:

The intrathecal reservoir with external management of morphine is a viable alternative for the management of severe cancer pain.
