Abstract

LOPEZ-CASTRUITA, Víctor Manuel; MANRIQUE-CARMONA, Luisa Piedad  and  REVUELTA-GUTIERREZ, Rogelio. Lidocaína tópica para prevenir el reflejo trigémino cardiaco durante cirugía de descompresión microvascular. Anest. Méx. [online]. 2017, vol.29, n.1, pp.3-7. ISSN 2448-8771.

Introduction:

Cardiac trigeminal reflex, is a reflection of the brain stem, which manifested sudden cardiac disturbances, including bradycardia, hypotension, apnea and asystole, gastric hypermobility.

Objective:

To assess the topical application of lidocaine to the 2% in the prevention of heart trigeminal reflection, during surgery of decompression microvascular.

Material and methods:

this is a controlled, clinical trial randomized. We included eleven patients undergoing microvascular decompression surgery, who were assigned to two groups. Group L with six patients who received lidocaine 2% and the SS Group with five patients who received saline. The form of administration in both groups, was of topical way in the surgical site. The main variable of the study was, frequency of cardiac trigeminal reflex, duration and severity.

Results:

The reflex trigeminal cardiac arose in one of eleven patients (9%) of the L group. No statistic differences were shown neither group. (p = 0.545), its duration was of 30 seconds and the severity was from the 20 to the 30%. Conclusions: This study seem to support the evidence that topical lidocaine 2% does not prevent the emergence of trigeminal cardiac réflex on microvascular decompression surgery, although strong evidence is required to issue definitive recommendations.

Keywords : lidocaine; trigeminal reflex cardiac microvascular decompression; trigeminal neuralgia surgery.
