The incidence of synchronous primary liver malign neoplasm is extremely low. We report a case of a 59 years old woman with Hepatitis B without surveillance, who was hospitalized for an incidental finding of 2 liver neoplasm. The patient underwent surgery (liver segmentectomy VII and VIII) and the pathology report described primary Hepatocarcinoma and primary Cholangiocarcinoma. The chronic hepatitis B increase the risk of malign primary liver neoplasm; thus, these patients should be regularly evaluated with image studies.
