Abstract

Anaplastic thyroid carcinoma is an infrequent tumor of thyroid. We report the case of a 60-year-old woman in whom the fine-needle aspiration biopsy was reported as Bethesda II. The discordance between imaging, clinical and surgical findings resulted in the patient undergoing total thyroidectomy with neck dissection. The final pathology reported a sarcomatoid anaplastic thyroid carcinoma T3bN1Mx, Ki-67 80%, positive for AE1/AE3, vimentin and peculiarly thyroglobulin, also was negative for thyroid transcription factor-1. The patient was alive 4 months after the initial surgery and 10 months from the initial symptoms.
