Glossopharyngeal neuralgia is a rare pain syndrome caused by compression of its cranial nerve, being commonly idiopathic. While vagoglossopharyngeal neuroalgia is even rarer. Manifested by pain accompanied by syncope and/or arrhythmia. We present the case of a 58-year-old man with nasal cavity cancer that metastasized to the right carotid space, encompassing the right internal carotid artery at 270°. Developing pain in the territory of the IX and hemodynamic instability, being managed with irradiation, and partially resolving the symptoms.
