Background: Carcinosarcomas (CS) or Mixed Mullerian Malignant Tumors (MMMT) represent 1% of malignant ovarian neoplasms. They are formed by an epithelium component and another mesenchymal one, both malignant.

Methods. A retrospective study was conducted on 8 cases of primary CS of the ovary in National Cancer Institute, Mexico from January 2005 to December 2008.

Results. CS or MMMT was found in 4.1 % of all the malignant primary tumors of ovary. The clinical initial presentation was both pain and abdominal expansion in 62.5 %, and a palpable mass and vaginal bleeding in 25 % of the patients. Surgery was performed on two (25%) of the patients, with residual tumor of less than 1cm, and chemotherapy was administered in 4/8 (50%). In 3/8 (37.5%) patients remained free of disease in the follow-up of 42 months, one (12.5%) with stable disease and four (50%) died from the disease.

Conclusions. Ovarian CS is a rare tumor, the clinical treatment of this neoplasm is aggressive, as it usually presents in advanced stages, with a poor prognosis with existing medical treatment.
