A male overview of 25 years old, who was admitted to the emergency room of the hospital, by accident on a motorcycle. The patient had impacted on a truck. The patient is a carrier of a rare syndrome called syndrome Richieri-Costa-Pereira, consisting of multiple malformations of jaw and larynx; the bones of the jaw do not bind, difficulties in oral communication. In more severe cases patients can not breathe properly and are often candidates for a tracheotomy. Upon arrival at the hospital the patient couldn´t be ventilated and, intubation failed, he was rescued with a laryngeal mask and was finally intubated through a retrograded intubation.
