Diagnosis of Deep vein thrombosis (DVT) is based almost exclusively in clinical findings, and the physical examination has a low sensitivity and specificity. Adequate integration of both parts with the presence of risk factors and the existence or not of an alternative diagnosis can be used to classify patients according to the Wells scale model. The D-dimer (DD) determination will indicate whether or not a Doppler Ultrasound is necessary. Phlebography remains the gold standard, although is reserved for specific cases. Anticoagulants at therapeutic doses for at least 3 months are the main management. Low molecular weight heparins have displaced unfractionated heparins in the treatment of this disease, due to the lower complications rates they present. The inclusion of new oral anticoagulants should optimize the ambulatory management of DVT.
