Major advances in intensive care units have resulted in survival of premature babies weighing <500 g or <23 weeks of gestational age. The same is true with premature infants with severe congenital malformations who would suffer grave sequelae and poor quality of life. This paper discusses the general condition of these children along with international regulations. We discuss the minimal indispensable care these infants require in regard to the principles exemplified in their best interest.
