Perioperative fluid therapy has been the subject of extensive discussion and study over the past decade. Starting from changing scientific ideas and concepts that attempt to explain and understand the behavior of fluids in the organism, we find ourselves since 1896 with Starling’s Principle explaining the physiology of fluid homeostasis inside and outside the endothelial surface layer (ESL) up to the present time, where thanks to the knowledge of the existence of glucocalix or also called “cell membrane protector” we have transformed and rationalized perioperative fluid therapy and left behind concepts such as the exaggerated replacement of perioperative losses and focused on therapy guided by objectives that optimize patient recovery and reduce postoperative complications.
