Fever in infants younger than three months without an evident source, can be part of a threatening or invasive bacterial infection with the risk of serious consequences for lack of adequate attention. This review article pretends to explore the definition of fever and invasive infections, the clinical and laboratory features as well as the series of protocols that gave origin to the low risk criteria that can be used to avoid excessive treatments or dangerous underestimations. The three most important low risk elements are: clinical setting where the absence of toxic appearance is essential, a normal urine analysis, and normal blood biomarkers, specifically white blood cells, C reactive protein and specially procalcitonin.
