Cardiac rhythm disturbances in neonates are usually benign and become symptomatic after the first seven days of life; clinical manifestations at the moment of birth are uncommon. We describe the case of a healthy, full-term female neonate presenting supraventricular tachycardia at the moment of birth and hemodynamic instability. Without structural heart disease, they usually have a good response to the medical treatment. Multiple pathophysiological mechanisms are involved. Supraventricular tachycardia is the most common of the tachyarrhythmias; the clinical assessment includes the identification and treatment of anemia, hydroelectrolytic disturbances, sepsis, respiratory diseases and structural alterations.
