Complex hernias with “loss of domain” are giant hernias with chronic wall defects. The separation of components is one of the techniques used in large abdominal hernias. We present the case of an 86-year-old female with a 43-year-old hernia with intestinal occlusion, laparotomy, anastomosis, adherenciolosis, and component separation with mesh placement; her evolution was favorable with satisfactory postoperative follow-up functionally without complications. The literature declares the surgeon’s own experience to be the most important prognostic factor for the outcome of the surgical intervention.
