The posterior fossa is an anatomic region susceptible to complications, its anatomical location, size and content of vital structures, making any change in its contents could be life threaning. Although tumors are more frequent in children, adults have a higher incidence of vascular lesions. During the perioperative period, preoperative assessment should include physical examination, with emphasis on neurological examination. The hemodynamic status of the patient and the position during the transsurgical period should be evaluated. Surgical objectives and transoperative incidences will give perspective on the planning of extubation. Knowledge of the anatomy, possible complications and risk factors will lead to adequate planning of the airway management, in order to reduce the rate of re-intubation and prolonged mechanical ventilation that results in higher morbidity and high costs of hospitalization.
