Phyllodes tumor of the breast is a rare neoplasm, representing < 1% of mammary tumors. We report the case of a 73-year-old woman with phyllodes tumor of 7 x 6 x 4 cm with pulmonary metastasis, who received six cycles of adriamycin, with complete response, and remained under surveillance. Breast phyllodes tumor presents with a median age of 45 years. Distant metastases in benign, borderline and malignant tumors represent 3.2%, 11.1% and 28.6% respectively, with lung, bone and abdomen being the most common sites. Metastases indicate a poor prognosis with an average survival of less than two years.
