Desmoid-type fibromatosis retroperitoneal is a rare, slow-growing, recurrent neoplasm without metastasis. The treatment of fibromatosis is currently proposed by active surveillance, however, due to the retroperitoneal location, there is a risk of mortality and losing the therapeutic option of active management. Being the established treatment, the complete or incomplete surgical resection. The subject is reviewed regarding the case in a 48-year-old man, a native of the central Peruvian jungle, without a traumatic history, with giant retroperitoneal fibromatosis. Being proposed to undergo incomplete resection followed by adjuvant with irradiation.
