Objective:

To present our center's experience in the assessment and management of biliary pathology with the use of third generation cholangioscopy.

Method:

A retrospective study was conducted at the gastrointestinal endoscopy department of the Hospital Regional General Ignacio Zaragoza, in Mexico City. All adult patients that underwent cholangioscopy within the time frame of March 2020 to March 2022 were included. Relevant demographic, clinical and biochemical patient characteristics were collected from their clinical records.

Results:

The study included 100 patients that underwent cholangioscopy. 52% of the patients were biliary stenosis with intent to biopsy in 36 (69.2%) cases. In all choledocholithiasis patients laser lithotripsy was applied. Stone clearance of the bile duct was successful in 93.7% of the patients. Procedure-related adverse events occurred in 4/100 (4%) patients and pancreatitis was the main complication.

Conclusions:

Cholangioscopy is a useful and safe tool for assessment and treatment of biliary strictures and choledocholithiasis, and should be used as standard of care during assessment of indeterminate biliary stenosis and treatment for difficult biliary duct stones.
