Primary mandibular intraosseous leiomyosarcomas are very rare malignant neoplasms. Regardless of their location, they are very aggressive tumors associated with high local recurrence and/or distant metastasis with very low survival. Complete surgical excision is associated with less recurrence and longer survival. Radiation therapy is effective in treating residual microscopic disease. We present the case of a patient with primary mandibular leiomyosarcoma who required a wide mandibular resection, achieving negative surgical margins and subsequent reconstruction treatment with a pectoralis muscle myocutaneous flap and a titanium plate.
