In April 2016, during a routine check, liver masses were found in a 54-year-old woman. Biopsy revealed a low grade neuroendocrine metastatic carcinoma (both synaptophysin and chromogranin positive). Elevated serum chromogranin and an octreotide scan showed liver lesions reinforcement. In September 2016, after starting systemic therapy, she developed progressive heart failure. Elevated transaminases, ammonium, B-type natriuretic peptide, and 5-hidroxy-indole acetic acid. A transthoracic echocardiogram showed severe tricuspid and pulmonary valve regurgitation. Diagnosis was carcinoid heart syndrome. Multidisciplinary approach and tricuspid valve replacement was performed. She restarted systemic therapy and later chemoembolization of liver metastasis. The patient was progression free until February 2022.
