Actinomycosis is a rare inflammatory condition, which can simulate benign or malignant abdominal pathologies. A 61-year-old male patient with abdominal pain and constipation. In the tomography with decreased lumen of the terminal ileum and collapse of the sigmoid colon. Colonoscopy with biopsy, reporting actinomyces grains. Management with ampicillin was started. When no improvement was observed, an exploratory laparotomy was performed, finding a tumor in the ileocecal valve (adenocarcinoma). Patient with poor evolution, he dies. Abdominal actinomycosis is a great simulator; we consider necessary to continue the study protocol to determine the existence of other underlying pathologies and avoid delay in diagnosis.
