Introduction: The obstruction of a prosthetic heart valve of the left side is a serious complication and the mortality is associated with the type of valve, duration of symptoms and emergency surgery. Case report: Male 55 years old, with a history of implantation of a mitral prosthetic heart valve type Saint Jude. He started with middle dyspnea on slight exertion, orthopnea and paroxysmal nocturnal dyspnea. During the cardiac examination was documented an arrhythmic heart sound and prosthetic click decreased in intensity. The levels of anticoagulation documented to international normalized ratio (INR) were 1.38. The echocardiogram showed a mean gradient of 25 mmHg and valve area of 0.7 cm2. Transesophageal echocardiogram showed an echodense image suggesting thrombus > 5 mm. The patient was undergone successful to valve replacement surgery. Conclusions: Surgery almost restores valve function in all patients and of nonfatal adverse event rates is small. The urgent surgery is safer and also perhaps more efficacious that the fibrinolytic therapy.
