Introduction:
An increased incidence of bile duct injuries has been reported following laparoscopic cholecystectomy, with the classic biliary injury being common bile duct transection. Roux-en-Y hepaticojejunostomy is the procedure of choice for biliary reconstruction and only a few laparoscopically treated cases has been reported.
Case report:
A 60-year old male who underwent laparoscopic fundoplication plus cholecystectomy showed a Bismuth I type biliary injury with common bile duct transection. The injury was diagnosed at the third postoperative day and was reconstructed at the sixth day after surgery using a laparoscopic Roux-en-Y hepatic-jejunostomy with fi ve ports. After 17 months, an open redo of the hepaticojejunostomy was required due to stenosis.
Conclusion:
Laparoscopic Roux-en-Y hepaticojejunostomy appears to be feasible and safe for a selected group of patients with biliary injuries. Patients like the one reported are ideal candidates to consider a laparoscopic approach.
