Introduction and objective:

Acute appendicitis is considered the most common cause of acute surgical abdomen, with a prevalence of 7% of the world population, with laparoscopic surgical treatment being the gold standard. However, at the beginning of the COVID-19 pandemic, a probable increased risk of contagion for the surgical team was described due to the use of the laparoscopy, which is why in our hospital we choose for open surgery for patients with acute abdominal pathology, we consider relevant to share our experience in a national reference center.

Case report:

A 47-year-old female patient, who started her symptoms with abdominal pain, nausea and vomiting, taking analgesic and antibiotic, who, after pain migration increased abdominal pain and exacerbation of pain, decided to go to a private doctor where a CT scan was performed, reporting acute appendicitis and images suggestive of SARS-CoV-2 infection, so she is sent to our hospital, where we decided to perform an open appendectomy and is sent to the Intensive Care Unit with a favorable evolution.

Discussion:

Currently the diagnosis of acute appendicitis in the context of a patient with COVID-19 represents a challenge since there are symptoms which belong to both diseases, so we consider it relevant to share our experience in a third level hospital. No relationship was found in the pathological study between coronavirus disease and acute appendicitis.
