The middle cerebral artery (MCA) is the larger of the two terminal branches of the internal carotid artery (ICA). There are different anatomical variations of the MCA, the most frequent are duplicated MCA, an accessory MCA, and fenestrated MCA. According to the classification of Teal et al. when the two vessels arise from the most distal portion of the internal carotid artery is called duplicated MCA. The term accessory MCA is used when the anomalous vessel arises directly from the anterior cerebral artery. Cerebral aneurysms are found more frequently in cases with anatomical variants such as accessory middle cerebral artery. Proper recognition of these anatomic variants by magnetic resonance angiography is vital for proper assessment and monitoring.
