Euthyroid sick syndrome (ESS) is a clinical entity that presents frequently in hospitalized patients, most importantly in critically ill ones. Since it was described, in the 1970s, it has attracted vast interest because it presents itself with a complex physiopathological mechanism. Changes in thyroid function tests (TFT) may be secondary to the acute state of illness. In all cases, we can find alterations in the activity of thyroid deiodinases, union of thyroid hormones to plasmatic proteins and secretion of thyroid stimulating hormone (TSH) and thyrotropin release hormone (TRH). It is known that its presentation, along with the lowering of T4 values to under 2 μg/dL, increases the risk of mortality in up to 80% in intensive care unit (ICU) patients. Hormone substitution with T3, T4, or both has not shown a concrete benefit, its prescription being controversial to this day. This is a systematic review that comprehends the main characteristics of the syndrome, physiopathology, clinical manifestations, treatment and differential diagnosis.
