Creutzfeldt-Jakob disease (CJD) is uncommon, and even rarer in pregnant women. Its anesthetic management is a challenge given the neurological alterations of these patients. We report the anesthetic management during the cesarean section of a pregnant woman with 34 weeks of gestation. The patient entered the operating room awake, but disconnected of the surroundings. The anesthetic management was done with total intravenous anesthesia with hemodynamic monitoring. Extreme transmission protection techniques for the health personnel were implemented and all the material used was disposed of. There were no transanesthetic incidents. Extubation was performed post-procedure based on the presence of airway protective reflexes. She was discharged without complications, with antipsychotic and anticonvulsant management. The product did not show congenital or neurological alterations. In the anesthetic management of a patient with CJD, it is necessary to consider biosecurity in the operating room.
