The diaphragmatic dysfunction induced by mechanical ventilation is the clinical expression of the alterations generated by the inactivity of the diaphragm in conditions of invasive mechanical ventilation. Various molecular pathways and mechanisms explain atrophy and subsequently functional disability as an important cause of failure in extubation in the context of the critically ill patient. In this subgroup of patients there is a causal relationship of early onset of muscle dysfunction and ventilatory support and that is mainly related to the use of control modalities, the presence of sepsis and multiorgan failure. We reviewed the concepts about diaphragmatic dysfunction, the mechanisms involved in its appearance and the resources for early diagnosis to initiate a timely rehabilitation and positively influence the prognosis of these patients.
