Pneumatosis intestinalis is a rare entity that is characterized by accumulation of gas in the submucosa and / or walls subserosa the digestive tract, usually associated with other diseases, with rare primary presentation. Clinical manifestations in most cases are asymptomatic, being uncommon presentation as acute abdomen, secondary to pneumoperitoneum spontaneous. We report the case of a patient with systemic sclerosis, who presented with a clinical picture of abdominal pain in the emergency department, with failure of conservative treatment and with progression to acute abdomen, for which surgical treatment was offered, resolving the secondary spontaneous pneumoperitoneum to intestinal pneumatosis that involved the entire small intestine with exploratory laparotomy and intestinal rest with excellent results.
