The retroperitoneum represents the main site of origin of 15% of soft tissue sarcomas; liposarcoma (LPS) is the most frequent of them. Complete resection with negative margins is the treatment of choice, being the dominant long-term survival predictor and decreasing local recurrence rates. The presence of abdominal compression symptoms at diagnosis denotes a large tumor size. We present a 45-year-old man with nonspecific abdominal symptoms and increased abdominal circumference of 3 months of evolution. A diagnosis of a well-differentiated, low-grade retroperitoneal LPS was made and subsequently underwent resection.
